For your convenience, we have placed some of our student forms on this page. A select few forms are also on the wall right outside of our office, located in Wilkinson Hall Rm# 316. All completed forms can be emailed to records@wau.edu or submitted in person to the office.

Students requesting exceptions to academic policies need to submit the Academic Petition request form to the Office of the Registrar. The form is most often used when students are desiring to take a course off campus after they have matriculated as a student. If this is the case the form must be submitted and approved prior to registering for any classes off-campus. It is recommended that seniors do not take any courses off-campus during their last semester.

For new Students: Please note that certain steps in the Admissions process will have to be completed before you will be able to set up your account. If you have completed all of the steps you will receive notification from the Office of the Registrar informing you that it is time to set up your account. Please wait until that e-mail has been received before trying to set up your account.
Change of addresses can be done in Self Service. However, it can also be done using the Change of Address form and submitting it to the Office of the Registrar.

If you wish to change your academic program by adding or removing a program (i.e. major, minor, pre-professional program) the change must be submitted officially to the Office of the Registrar using the Change of Academic Program form

If you have had a legal name change and need to officially change the name in our system you will need to submit the Change of Name Request form to the Office of the Registrar

* Before we will change your name, you must submit one of the following items: a copy of your driver’s
license, passport, social security card, marriage license or other legal document showing that the name has
been changed through a legal process.
If graduation candidates need to have verification that they have completed their degree requirements prior to when their degree conferral has occurred, they can request a Completion Letter from the Office of the Registrar.

If you are a former graduate of the university and you have a need to obtain a replacement diploma you may request one be reissued by submitting the form below to the Office of the Registrar.

Fee: $50
Note: Prior to a diploma reissue, the Office of the Registrar must verify your degree information to ensure accuracy. Reissued diplomas adhere to the current format (i.e. current name of university), and they bear the signatures of the current university officers, NO EXCEPTIONS. A replacement statement appears on all reissued diplomas.
A Graduation Application must be submitted to the Office of the Registrar by the deadline stipulated in the academic calendar to initiate the process of degree conferral.

The Graduation Invitation Letter is for those who have loved ones who live outside of the United States and cannot leave their country freely without such a letter.
Note: These letters are NOT graduation tickets.

An Incomplete (“I”) indicates that a student’s work in the course is incomplete
because of illness or unavoidable circumstances and not because of negligence
or inferior performance.

Process:
The student, in consultation with the professor, requests an “I” grade by
completing the appropriate form from the Office of the Registrar prior to the
final examination period. Signatures of the instructor and department chair
are required. The instructor will designate on the form the time limit (not to
exceed eight weeks into the next semester).
A default grade of “F” will be recorded after the designated date indicated on
the incomplete request form has expired, unless the instructor has submitted
a change of grade prior to the indicated deadline. The instructor must approve
any request for an extension beyond the time period allowed prior to the
designated deadline and a new Incomplete request form must be submitted
with the new deadline stipulated
To withdrawal from the university students must submit a University Withdrawal Form to the Office of the Registrar. Students are encouraged to speak to the Betty Howard Center for Student Success if they are contemplating withdrawing from the university where they will guide you through the process.
